About this Position

The Licensed Practical Nurse (LPN) / Phlebotomist / Collaborative Care Manager is responsible for delivering high-quality, patient-centered care through a combination of clinical nursing, phlebotomy, and care coordination services. This role ensures continuity of care, supports behavioral health integration, and enhances patient outcomes through proactive engagement and collaboration with interdisciplinary teams.

Required Qualifications

    • Licensed Practical Nurse (degree must be conferred on or before agreed upon start date)
    • Phlebotomist Vocational/Technical
    • Minimum 2 years of clinical nursing and/or phlebotomy experience.

Certifications, Registrations, and/or Licenses:

  • LPN license in the State of Oklahoma.
  • Valid Driver’s License

Skills, Proficiencies, and/or Knowledge:

  • Proficient in venipuncture and capillary collection techniques.
  • Knowledge of specimen handling, labeling, and processing protocols.
  • Strong infection control practices and proper use of PPE.
  • Familiarity with phlebotomy tools (e.g., tourniquets, vacutainers, lancets, butterfly sets).
  • Proficiency in EHR documentation and data retrieval.
  • Ability to assess physical and behavioral health needs.
  • Understanding of medication regimens, side effects, and adherence strategies.
  • Skilled in navigating complex healthcare systems and connecting patients to services.
  • Familiarity with behavioral health screening tools (e.g., PHQ-9, GAD-7).
  • Knowledge of common behavioral health conditions and treatment approaches.
  • Ability to recognize and respond to urgent behavioral health needs.
  • Strong organizational, communication, and time management skills

Preferred Qualifications

Certifications, Registrations, and/or Licenses:

  • Certified Phlebotomy Technician (CPT) preferred

Skills, Proficiencies, and/or Knowledge:

  • Bi-lingual highly preferred
  • Experience with EPIC highly preferred
