About this Position

THIS IS A GRANT FUNDED POSITION 

As a member of the multidisciplinary health care team, the Clinical Case Manager assesses, plans, implements, coordinates, monitors, and evaluates options and services to meet an individual’s health care needs through communication and available resources to promote quality cost-effective outcomes.  The Clinical Case Manager facilitates access to all core medical services and coordinates comprehensive HIV primary care service delivery planning for patients through assessment, service facilitation, referral, advocacy, and follow-up. By working collaboratively with the patient and the health care team, and utilizing established Levels of Service Criteria, the Clinical Case Manager plans and implements with the patient a Comprehensive Medical and Health-related Services Care Coordination Plan, updates the plan as needed, documents all care and service contacts, monitors patient progress, and continually reassesses the individual's need for level of service. The Clinical Case Manager provides treatment adherence readiness counseling and treatment education and support, in collaboration with the other members of the health care team. 

Required Qualifications

  • Master's level degree in Counseling Psych (MS), Master's level degree in Social Work (MSW) or an undergraduate degree in Nursing (BSN required). Other equivalent educational qualifications/experience combinations will be considered (degree must be conferred on or before agreed upon start date)

Certifications, Registrations, and/or Licenses:

  • For RN, possession of valid temporary or permanent professional nursing license issued by the Oklahoma State Board of Nurse Registration and Education is required).

 

Skills, Proficiencies, and/or Knowledge:

  • Familiarity with local health and social services care coordination, including eligibility assistance, is essential.  
    Candidates must demonstrate excellent interpersonal communication abilities, proficient written planning and documentation skills, and a proven track record of working effectively with vulnerable populations.
  • Local travel within the 918 area of Oklahoma is necessary, along with reliable transportation and an understanding of the needs of rural communities, communities of color, and LGBTQ+ individuals, as well as those affected by substance use, incarceration, or homelessness.  

Preferred Qualifications

  • For MS/MSW, Licensure is preferred (LCSW or LPC).
  • Two to five years professional experience in health and human services setting


Skills, Proficiencies, and/or Knowledge:

  • Previous case management experience preferred
